This intake form is the first step to the onboarding process. This intake form is required for all potential clients to fill out beforehand. Once completed we will contact you to set up a “Discovery Session”  A free fifteen minute session via telehealth zoom or phone. At
that time, we will decide together if we are a good fit. Assuming all goes well, we will move forward setting up a “Kick Off Appt” . You will be required to fill out  more detailed personal intake form called “ Confirmed Coaching Client Intake Form “, Or ”Reiki Energy Healing  Intake Form”, as well as our  “Contract “and “Payment Agreement” to be signed by both Client and Coach or Practitioner before our “Kick off Appt” (Online  Signature is acceptable ). We will email you these forms ahead of time for your convenience.
